Summary: This position supports Mercy's philosophy of patient centered care by providing comprehensive pharmaceutical care services to patients and employees. The pharmacist will provide clinical support patient education dispensing and monitoring services for high-cost and specialty medications as part of the patients interdisciplinary healthcare team. Services may be provided via face-to-face and telehealth encounters.

Job Description:

Job Duties:

• Provides medication therapy management and therapeutic drug monitoring for high-cost and specialty medications
• Completes a comprehensive patient assessment evaluating the patient prescription for appropriateness including indication dose drug interactions and adverse drug reactions
• Communicates effectively with ordering prescribers regarding recommendations for modifications to patient medication therapy to achieve care goals
• Facilitates effective communication with a diverse population of patients and employees in-person via telephone and/or via electronic health record
• Provides education to patients and staff
• Identifies and reports patient safety issues related to medication use and works on resolutions
• Accurately and efficiently documents all appropriate information in the electronic health record
• Assists with special projects pertaining to specific area of interest and/or expertise
• Demonstrates evidence of continued professional development
• Ensures that the pharmacy delivers quality services in accordance with pharmacy policy and standards of work and the laws rules and regulations that govern the distribution of drugs
• Accurately performs opening and closing the pharmacy operating the cash register entering patient insurance information into the computer and performing on-line prescription adjudication as needed
• Responsible for intra-departmental communication and promotes effective working relations to facilitate the department's ability to meet its goals objectives patient care and business needs
• Follows Mercy's safety guidelines carries out job-specific safety duties and responsibilities and promptly reports any unsafe conditions situations incidents and injuries

Knowledge Skills and Abilities:

• Ability to meet the pharmaceutical care needs of the following patient age groups: neonatal pediatric adolescent adult and geriatrics.
• Excellent interpersonal communication skills. Must be able to talk with others to obtain and provide information in a professional manner.
• Ability to operate basic computer applications and complete projects in a timely manner.
• Ability to calculate dilution and dosing problems.

Professional Experience: Successful completion of an American Society of Health-System Pharmacist (ASHP) accredited Post-Graduate Pharmacy Residency (PGY-1) preferred. Successful completion of an ASHP accredited PGY2 Residency in Ambulatory Care preferred. Twelve months of community/ambulatory care pharmacy practice preferred.

Education: Doctor of Pharmacy (PharmD) or Bachelor of Science (BS) degree in pharmacy required.

Licensure Certification Registration: Must have current Iowa Pharmacist License or be eligible for Iowa Board of Pharmacy pharmacist licensure. Employment is contingent upon being licensed by the Iowa Board of Pharmacy by start date. Board Certified Pharmacotherapy Specialist (BCPS) or Board-Certified Ambulatory Care Pharmacist (BCACP) preferred. Current certification in Basic Life Support in accordance with the American Heart Association preferred. Iowa Dependent Adult Abuse/Iowa Child Abuse Mandatory Reporter training required. Employee must submit certification of completed training to Human Resources prior to start or transfer date.
