Ambulatory Care Pharmacist- Full Time - Oncology - Murrieta

Provides ambulatory pharmaceutical care services using approved protocols in collaboration with other health professionals, including managing patient drug therapy, providing patient education, and preparing and/or overseeing the preparation and dispensing of medications. Promotes high quality, evidence-based, cost-effective pharmaceutical care. Performs these duties in the clinical areas assigned.

Essential responsibilities include upholding Kaiser Permanente's policies and procedures, conducting patient medication history inquiries, synthesizing treatment plans, identifying and resolving medication-related problems, coordinating laboratory monitoring of drug therapy, providing patient education, and performing drug utilization review.

Basic qualifications include a minimum of 1200 hours experience in the last five years working under collaborative drug therapy management protocol or successful completion of an ASHP Accredited PGY-1 residency. Additionally, a qualified candidate should have at least one of the following credentials or experiences: Board Certified Oncology Pharmacist (BCOP), completion of PGY-2 in oncology practice, or completion of 1000 hours in sterile compounding oncology pharmacy with direct clinical protocol management.

Education requirements include a degree in pharmacy meeting minimum mandatory educational requirements for licensure. License, certification, and registration requirements include a Pharmacist License (California) and Basic Life Support within three months of hire.
