Your responsibilities as a Licensed Practical Nurse (LPN) will include:

• Providing in-home direct patient care as per the prescribed care plan, while monitoring the patient's progress.
• Participating in ongoing assessments and adjustments to care as required.
• Documenting patient conditions accurately and reporting any changes (vitals, blood tests, etc.).
• Administering medications and insulin with accurate documentation.
• Caring for wounds, changing dressings, and assisting with personal hygiene tasks.
• Assessing patients for further issues like infections or injuries and providing necessary treatment.
• Supporting RNs or physicians in specialized procedures.
• Educating patients and their families about health-related needs.
• Completing timely and thorough clinical documentation regarding patient progress.
• Collaborating with the healthcare team to ensure optimal patient outcomes.
• Communicating effectively with the interdisciplinary team to coordinate care and assist with discharge planning.

To qualify for this role:

• You must possess a valid driver's license and auto liability insurance if driving is required.
• Graduation from an accredited nursing program is essential.
• A current valid nursing license in the U.S. is required.

Working conditions and physical requirements:

• Regular travel within the designated service area is expected.
• Work is typically conducted in patients' homes, schools, or other occupational settings.
• You will encounter exposure to communicable diseases and other safety hazards, requiring adherence to safety protocols and use of protective equipment.
• The role involves frequent lifting, repositioning, and transferring of patients.
• Medium physical activity is involved, occasionally requiring lifting or moving up to 50 lbs.
