BAYADA Home Health Care is currently seeking a Registered Nurse (RN) to join the Fort Collins Visits team. This office works with adult and senior clients on a per visit basis in homes throughout Larimer and Weld Counties.

• Coverage Territory: Weld and Larimer Counties
• Schedule: Per Diem (+ Weekend Rotation)

One-year prior clinical experience as a Registered Nurse is required; Home Health experience preferred, but not required.

As a home health nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.

• Monitor clients' conditions
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments
• Case management and coordination
• Accurately document observations, interventions, and evaluations
• Travel to clients in your local area on a schedule that fits your needs

Qualifications for a BAYADA nurse:

• A current Colorado nursing license in good standing
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
