Role Overview:

As a Medical Social Worker, you will play an essential role in supporting patients and their families at Williamson Medical Center. Your work will involve providing short-term counseling and crisis management for individuals adjusting to illness, lifestyle changes, or financial concerns related to their diagnosis and hospitalization. Key responsibilities include:

1. Assessing and managing psychosocial, alcohol and drug problems.
2. Conducting discharge planning in collaboration with physicians, staff, patients, families, and community agencies.
3. Facilitating effective communication and development of discharge plans to reduce anxiety for patients and families.
4. Working within regulations related to CMS, Commercial Payers, and Bundle programs.

Key Requirements:

• Licensure by Tennessee Health Related Boards in Social Work; LCSW preferred.
• Master's degree in social work or a comparable Master's level degree.
• Experience in a hospital or clinical setting.
• Strong verbal and written communication skills.
• Familiarity with computer systems, especially Meditech and Allscripts Care Management.
