Job Summary

The Licensed Practical Nurse (LPN) plays a crucial role in providing professional nursing care for residents of all ages in long-term care settings. Under the supervision of a registered nurse, advanced practice provider, or physician, you will apply your knowledge of nursing principles to ensure the health and safety of our residents.

Key Responsibilities:

• Assess residents’ conditions and administer medications or treatments as prescribed.
• Maintain accurate documentation of care provided, including medications and resident behaviors.
• Collaborate effectively with interdisciplinary care teams to enhance resident health outcomes.
• Communicate clearly and maintain confidentiality while counseling residents and their families about prevention and treatment plans.
• Participate in care planning while utilizing appropriate age and population-specific standards.
• Provide technical support to healthcare professionals as necessary.

Qualifications

• Graduation from an accredited practical nursing program and a valid Licensed Practical Nurse (LPN) license.
• Associate degree in practical nursing is preferred.
• IV therapy/management certification may be required depending on specific duties.
• A minimum of 400 hours of experience as an LPN in a clinical setting is highly valued.
