Your Role as a Case Manager

• Collaborate with an interdisciplinary team to create patient-centered treatment plans based on individual needs and preferences.
• Develop tentative discharge plans and prepare contingencies to ensure smooth transitions.
• Monitor patients’ experiences, ensuring quality and appropriateness of services.
• Facilitate weekly team conferences and manage treatment plan adjustments.
• Conduct assessments promptly and educate patients and families about the case management process.
• Assist in the procurement of necessary resources to avoid discharge delays.
• Ensure compliance with regulations while making timely referrals to post-discharge services.

What You Need to Join Our Team

• Valid license or certification in your discipline (RN, SW, OT, PT, ST, or Rehabilitation Counseling).
• For nursing professionals, a minimum of an Associate Degree in Nursing is required; BSN is preferred.
• A minimum of two years of rehabilitation experience is preferred.
• Certification as CCM® or ACM within two years of entry into the position is necessary.
