Your Role as a Case Manager

• Collaborate with an interdisciplinary team to guide treatment plans based on patient needs.
• Establish and communicate discharge plans while monitoring patient experience for quality and compliance.
• Conduct weekly team conferences to coordinate treatment modifications.
• Engage with patients and families promptly to establish care goals and support.
• Ensure accuracy in discharge documentation and facilitate timely referrals post-discharge.

Qualifications

Licenses or Certifications:

• Must be qualified to complete assessments within your discipline (RN, SW, OT,PT, ST, or Rehabilitation Counseling).
• If licensure is required, you must hold an active license.
• Certifications such as CCM® or ACM are required within two years of entering the position.

Minimum Qualifications:

• For Nursing, an Associate Degree with RN licensure is required; BSN preferred.
• Other eligible healthcare professionals must possess a minimum of a bachelor's degree, with a graduate degree preferred.
• Preferred two years of rehabilitation experience.
