Become the Case Manager You Aim to Be

• Collaborate with an interdisciplinary team to develop patient-centered treatment plans.
• Oversee discharge planning and prepare contingency plans as needed.
• Facilitate weekly team conferences and coordinate treatment modifications.
• Complete all required documentation, including case management addendums.
• Stay informed on relevant regulations, company policies, and operational standards.
• Review case management reports and implement appropriate actions.
• Engage with patients and families within 24 hours of admission to establish care plans.
• Educate families about rehabilitation processes and the Case Manager’s role.
• Ensure timely referrals and manage resource procurement to prevent discharge delays.
• Participate in ongoing quality assurance and compliance processes.
• Be part of the on-call rotation as necessary.

Qualifications

Licensure/Certification:

• License required for your discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
• Hold an active state license if required; must meet requirements for CCM® or ACM certification within two years of starting.

Minimum Qualifications:

• For Nursing, an Associate Degree in Nursing required; RN licensure is essential, with a BSN preferred.
• For other licensed or certified healthcare professionals, a bachelor’s degree is required, and a graduate degree is preferred.
• Minimum two years of rehabilitation experience is preferred.
