Pay: $33.82 - $50.73 / hour. Pay is dependent on applicant's relevant experience.

Summary:

We are seeking a dedicated Palliative Care Clinical Social Worker to join the University of Colorado Hospital Palliative Care Service. In this role, you will provide essential support to patients and families through values-based goals of care discussions, advance care planning, and transitions of care for those utilizing palliative care services. You will assess patients and their families, coordinate necessary interventions, and facilitate grief support, helping to address the physical, social, emotional, economic, and environmental challenges faced by patients.

Responsibilities:

• Deliver specialty-level palliative care psychosocial support to patients through direct counseling and collaborative visits, exhibiting cultural humility and sensitivity.
• Conduct and document thorough palliative care psychosocial assessments.
• Collaborate with and support social worker team members in delivering services to palliative care patients.
• Participate in patient-family meetings to assist in values-based goal setting and care planning.
• Identify and coordinate outpatient services relevant to palliative care patients.
• Lead educational and outreach initiatives related to the social services-palliative care interface.
• Utilize critical thinking skills and decisive judgement while working independently in a fast-paced environment.

Requirements:

• Master's degree in Social Work.
• State licensure as a Licensed Clinical Social Worker (LCSW).

Preferred:

• Commitment to obtaining Palliative Care certification within 2 years.
• Minimum of 2 years clinical experience in a hospital setting, with a strong preference for palliative care experience.
