• WA state license; active, unrestricted
• Board Certified Obstetrics & Gynecology; Board Eligible (2-5 years post training).
• DEA

Delivery volume over the last 2 years.

NPDB self-inquiry report: The NPDB - Home Page (hrsa.gov) + explanations (if applicable).

Updated CC (with MM/YY; all gaps over 30-days explained)

14 shifts per month of availability (weekdays + weekends).

COMPENSATION:

• Competitive Market Rate (rate based on availability, experience, certifications, housing, and transportation needs, etc.)
• Malpractice insurance provided.

SCHEDULE:

• 24-hour call. 14 shifts per block.
