Responsibilities
The Manager of Case Management is responsible for assisting the Director of Case Management with hospital case management, social work, transition management, and operational management of the case management department.
Qualifications
Job Requirements:

RN, Associate's degree in nursing or higher.
1+ years' experience in a case management leadership role.

Preferred Job Requirements:

2+ years' experience in acute hospital-based case management.
Certification in case management (ACM or CCM).
