Location : Park Ridge Living Center, Clifton Nursing Home, DeMay, Edna Tina Wilson Living Center, Unity Living Center

Hours Per Week : 36 to 40 Hours

Sign-On Bonus : Up to $10,000 (based on location)

SUMMARY : As a Licensed Practical Nurse, you will play a vital role in providing compassionate and professional care to our long-term and post-acute residents. You will be responsible for leading and overseeing the care provided by Certified Nursing Assistants (CNAs), while ensuring meticulous documentation and reporting are upheld. Your leadership will greatly enhance the quality of care for our patients.

RESPONSIBILITIES :

• Administer medications accurately and on time, ensuring all documentation is complete.
• Perform treatments and document progress according to physician orders and facility protocols.
• Maintain precise and thorough resident care documentation using our computerized medical record system.
• Effectively communicate relevant reports, both written and verbal, to appropriate staff members.
• Monitor resident conditions and report any significant changes to charge nurse/supervisor and/or physicians promptly.
• Prepare and review medication administration records in accordance with facility protocols.
• Complete necessary Minimum Data Set (MDS) documentation and observations in a timely manner.
• Support the CNA team by assisting with direct activities of daily living (ADL) care for residents.

REQUIRED QUALIFICATIONS:

• Graduate from an accredited School of Practical Nursing
• Current licensure in State of New York

PREFERRED QUALIFICATIONS :

• One year of experience preferred

EDUCATION:

LICENSES / CERTIFICATIONS: 

BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
