NEW GRAD Case Manager. $55/hr

Salary: $55 / Hour

RN Case Manager & NEW GRADS Encouraged!

Rate: $55/hr (Includes Weekly Pay & Full Benefits Offered)

Shift: Monday to Friday, 8a - 4p

Temp to Perm position, Contract to Direct Placement

NYS RN License, no prior experience required

The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime.

Minimum Requirements:

• Current RN licensure in state practicing

• At least one year of Case Management experience preferred

• Current CPR if applicable

• TB questionnaire, PPD or chest x-ray if applicable

• Current Health certificate (per contract or stateregulation)

• Must meet all federal, state and local requirements

• Must be at least 18 years of age
