Key Responsibilities:

Provide comprehensive wound care to patients in various settings, including homes and clinics, ensuring optimal healing and comfort.
Assess, evaluate, and manage patients' wound care needs, developing personalized treatment plans.
Collaborate with a multidisciplinary team to coordinate patient care and ensure the best outcomes.
Educate patients and their families on wound care management and prevention strategies.
Maintain accurate and timely documentation of patient care and progress.

Qualifications:

Must have a Woundcare certification
Current and unrestricted Nurse Practitioner license in the state of California.
Certification in wound care or relevant experience is preferred.
Excellent clinical skills and the ability to work independently.
Strong communication and interpersonal skills.
Valid driver's license and reliable transportation for travel within the Thousand Oaks area.
