Job Summary:
A Medical Scribe is responsible for accurately and efficiently documenting patient encounters in the electronic health record (EHR) under the direct supervision of a physician or other healthcare provider. The scribe's primary role is to assist the provider in navigating the EHR, ensuring all information is correctly captured, and maintaining a high level of productivity during patient visits. This non-clinical role is crucial for improving physician efficiency, reducing documentation burden, and allowing providers to focus on direct patient care.
Key Responsibilities:
Workflow Support: Assist the physician in navigating the EHR system, including locating patient charts, lab results, imaging reports, and other relevant data. Anticipate the physician's needs during the patient encounter, such as preparing orders, prescriptions, and referral forms. Help manage the physician's schedule and patient flow to maximize efficiency. Compliance and Confidentiality: Adhere strictly to all HIPAA (Health Insurance Portability and Accountability Act) regulations and maintain patient confidentiality at all times. Follow all clinic and hospital policies and procedures regarding documentation, patient privacy, and data security. Communication: Communicate effectively with physicians, nurses, and other healthcare staff to ensure a smooth and efficient workflow. Clarify any ambiguities in documentation with the physician to ensure accuracy. Professional Development: Maintain a working knowledge of medical terminology, anatomy, and common medical conditions. Continuously seek to improve efficiency and accuracy in documentation.

- Documentation:
- Accompany the physician into patient rooms to accurately document the patient's history, physical examination, assessment, and plan in real-time.
- Transcribe dictated reports, physician notes, and other medical information into the EHR.
- Ensure all entries are clear, concise, and medically accurate, using appropriate medical terminology and abbreviations.
- Review and verify all documentation for completeness and accuracy before the physician's final review and signature.
