As a Master Social Worker in Care Coordination, you'll play a key role in delivering comprehensive care coordination services to assigned patients. This position is responsible for ensuring the clinical quality of services providedboth directly and through collaboration with otherswhile identifying and addressing any barriers to effective patient care.

Our Case Managers support various hospital units and typically rotate through multiple areas each day. Core responsibilities include:

• Conducting initial patient assessments
• Participating in daily interdisciplinary rounds
• Communicating discharge plans and updates to patients, families, and the care team
• Delegating appropriate tasks to Transitional Care Associates

This is a dynamic role that requires strong communication, critical thinking, and a patient-centered approach to care.

This is a full-time opportunity working in adult health working five-8 hours shifts. Expected hours are 8AM to 4:30PM or 8:30AM to 5:00PM. There are weekend and holiday requirements for this role. Enjoy a flat rate $3/hour weekend shift differential.

This position provides comprehensive care coordination for patients as assigned. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for clinical quality of Care Coordination services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care. The goal is to empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care of the population that it serves which includes planning for a safe discharge, continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.

Core functions include managing individual patients across the health care continuum to achieve optimal outcomes, acting in a leadership function with process improvement activities, collaboratively developing and managing the interdisciplinary patient discharge plan, maintaining knowledge of Medicare, Medicaid and other program benefits, establishing and promoting a collaborative relationship with physicians, payers, and other members of the health care team, educating internal members of the health care team on case management and managed care concepts, and having freedom to determine how to best accomplish functions within established procedures.

Minimum qualifications include a Master's Degree in Social Work, Counseling or related field, a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW), Basic Life Support (BLS) certification for acute care settings, and proficiency level typically achieved with 2-3 years clinical experience. Preferred qualifications include Certification for CCM (Certified Case Manager) and additional related education and/or experience.
