Responsibilities

Leads and directs case management services, workflow and resources.

  Formulate and implement operational strategies and initiatives in the case management program.

  Direct and evaluate all services provided, including care plan development, service leveldetermination, and complaint management to achieve performance and quality control objectives.

  Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes and issues.

• n

  Successfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.

Requirements

Licensure / Certification / Registration:

One or more of the following required:

  Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job
