This is a full-time position with weekday hours from 8 AM to 5 PM. You'll be on-site at an assigned hospital daily from 9 AM to 3 PM, with remote work for the remaining hours.

The Manager of Acute Care Services plays a pivotal role in ensuring outstanding patient outcomes and managing quality care across the healthcare continuum. You will advocate for patients and their families, assisting them as they navigate through various healthcare systems and providers.

As a vital member of our Complex Care Team, you will develop effective care plans, leveraging available resources and collaborating with other healthcare professionals to optimize patient care. Your responsibilities include building relationships with patients, families, physicians, and various care facilities to facilitate timely and safe discharges and support post-hospital care.

Key Responsibilities:

• Maintain a daily presence at assigned hospitals and manage a balanced caseload.
• Identify opportunities for improved allocation of healthcare resources and determine inpatient versus observation status for patients.
• Assess patient safety risks, functional status, and knowledge deficits to enhance self-management.
• Collaborate with the Complex Care and Clinical Strategy Teams to implement our standard operating procedures effectively.
• Conduct hospital discussions to introduce patients to our Care Management/Disease Management programs.
• Identify and address potential barriers to safe discharge from the very first day of hospitalization.
• Work with specialists to care for patients in healthcare facilities as needed.
• Coordinate transitions for patients at Skilled Nursing Facilities (SNFs) to ensure appropriate levels of care.
• Facilitate communication of discharge plans with all stakeholders involved in patient care.
• Document patient discharges and keep all necessary parties informed.
• Conduct Social Determinants of Health screenings to identify needs for community or social services support.
• Engage patients in discussions about our Chronic Care Management (CCM) and Disease Management (DM) programs.
• Arrange for timely follow-up appointments with primary care providers.

Qualifications:

• Strong communication and interpersonal skills, capable of working with diverse community groups.
• Critical thinking and organizational skills for effective patient care planning.
• Proficiency in Microsoft Office Suite and various healthcare documentation systems.
• Current and active Registered Nurse (RN) license is required, along with relevant clinical experience.
• Bachelor's Degree in Nursing (BSN) preferred, with at least two years in clinical settings and experience in utilization review or case management.
