SUMMARY:

Join our dedicated team as a Licensed Practical Nurse (LPN) in providing compassionate and professional patient care to residents in long-term and post-acute care. You will play a crucial role in leading and supervising the care provided by Certified Nursing Assistants (CNAs) while ensuring accurate documentation and reporting of patient progress.

Our facility, DeMay Living Center, is a 180-bed skilled nursing facility connected to Newark-Wayne Community Hospital. We offer 24-hour care to individuals and families requiring rehabilitative, complex post-acute, transitional, neurobehavioral, and long-term care. Our environment is safe and supportive for those experiencing dementia symptoms.

RESPONSIBILITIES:

• Administer medications timely and accurately while ensuring proper documentation.
• Perform treatments and document residents' progress as per physician orders.
• Ensure thorough and concise resident care documentation using a computerized medical record system.
• Communicate relevant updates and reports to appropriate staff following department protocols.
• Monitor residents’ conditions and report significant changes to the charge nurse or physician promptly.
• Complete medication administration records pre-reviews following facility protocols.
• Conduct required MDS documentation and related observations efficiently.
• Assist CNAs in providing direct Activities of Daily Living (ADL) care to residents.

REQUIRED QUALIFICATIONS:

• Graduation from an accredited School of Practical Nursing.
• Current licensure in the State of New York.

PREFERRED QUALIFICATIONS:

• One year of experience in a long-term care setting preferred.

LICENSES / CERTIFICATIONS:

• BLS - Basic Life Support - American Heart Association (AHA)
• LPN - Licensed Practical Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

This position entails medium work, requiring the exertion of 20 to 50 pounds of force occasionally, and/ or 10 to 25 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects. Frequent walking, standing, and squatting are essential.
