Job Summary:
We are seeking a Licensed Master Social Worker (LMSW) to provide behavioral health services, including assessment, treatment, care coordination, and collaboration with an integrated clinical team. This hybrid role combines on-site and remote work.

Key Responsibilities:

• Assess patients using evidence-based tools and provide behavioral health treatment (CBT, Motivational Interviewing, crisis intervention, etc.).
• Collaborate with care teams to develop treatment recommendations and monitor progress.
• Provide crisis intervention and engage clients with limited utilization of behavioral health services.
• Document services accurately in electronic health records and participate in supervision, team meetings, and case conferences.

Qualifications:

• Master of Social Work (MSW) and active LMSW licensure.
• Proficiency in biopsychosocial assessments and DSM-V diagnoses.

• Skilled in risk assessment and care coordination.

This position offers a hybrid schedule - 1 day on-site per week and the rest remote.
