MedAdventures is seeking a skilled and compassionate Physical Therapist (PT) to join our Home Health team serving Macedonia, OH and surrounding communities. This is a rewarding opportunity to provide essential therapy services in patients’ homes—helping them regain mobility, function, and independence while remaining in a familiar and comfortable environment.

Responsibilities:

Provide physical therapy services to home-bound patients in accordance with individualized care plans
Complete comprehensive evaluations and develop goal-driven treatment plans
Document visits and progress notes accurately and in compliance with OASIS and Medicare guidelines
Collaborate with nursing, case management, and other disciplines to ensure coordinated care
Educate patients and caregivers on exercises, safety strategies, and treatment goals
Manage a caseload efficiently within the assigned territory

Qualifications:

Active Ohio PT license (or eligibility to obtain)
OASIS experience required
Prior home health experience strongly preferred
Strong clinical reasoning, communication, and time-management skills
Reliable transportation, valid driver’s license, and proof of auto insurance
