Physical Therapist (PT) – Skilled Nursing Facility (SNF)

MedAdventures is seeking a compassionate and motivated Physical Therapist (PT) to join a trusted Skilled Nursing Facility (SNF) team in Bennington, VT. This is a great opportunity for a PT who’s passionate about helping patients regain strength, mobility, and independence in a supportive, team-based care setting.

You’ll work closely with an interdisciplinary team to deliver evidence-based therapy and help residents improve their quality of life through tailored treatment plans.

Key Responsibilities:

Evaluate residents and develop individualized treatment plans to restore function, reduce pain, and improve mobility
Deliver skilled physical therapy services in accordance with physician orders and facility protocols
Monitor progress, reassess goals, and adjust treatment plans as needed
Educate patients, caregivers, and staff on exercise programs and mobility techniques
Document all care provided accurately and in a timely manner

Qualifications:

Active Vermont PT license (or eligibility to obtain)
Experience in a skilled nursing or long-term care setting preferred
Strong interpersonal, clinical, and documentation skills
Ability to collaborate effectively with an interdisciplinary team
Commitment to patient-centered, outcome-driven care
