About the Role:

We are seeking experienced and compassionate Nurse Practitioners (NPs) to join a home-based locum tenens program serving health plan members across Michigan. This role is ideal for providers who value autonomy, flexible scheduling, and meaningful one-on-one patient care.

This position involves in-home clinical visits to health plan members—primarily seniors (65+) and select Medicaid populations (18+). You’ll play a key role in closing quality gaps, performing diagnostic screenings, and providing comprehensive assessments that inform future care planning.

Key Details:

• Location: Travel throughout Michigan (with a focus on Kent County and other regions as needed)
• Start Date: October 15, 2025
• Initial Contract: Through December 31, 2025 (extensions likely)
• Schedule: 5 days/week (flexible: Mon–Fri preferred, but Wed–Sun accepted)
• Hours: 8-hour shifts within 8:00 AM–8:00 PM window
• Daily Visits: 5–6 members/day (up to 8 scheduled to account for cancellations)

Responsibilities:

• Conduct solo in-home visits to members for:
• Level 2–3 Diagnostic Visits (e.g., A1C, FIT, KED, retinal exams)
• Level 4 Comprehensive Annual Wellness Exams
• Perform diagnostic screenings and collect labs
• Assess Social Determinants of Health (SDOH)
• Collaborate with care coordination teams to deliver 30-day resource guides for members
• Complete documentation via iPad/EHR platform (training provided)
• Ensure clinical safety — report any unsafe situations to coordination team

Qualifications:

• Licensure: Active Nurse Practitioner license in Michigan (with MI RN license)
• Certification: Board Certified FNP-C or GNP (Family Practice, Internal Medicine, or Geriatrics)
• Payer Enrollment: Enrolled as a Medicare and Medicaid provider in Michigan
• Experience:
• New grads with Home Health RN experience considered
• Strong preference for in-home assessments and diagnostic experience
• Familiarity with HEDIS, HRAs, risk adjustment, and quality-gap closures
• Tech Savvy: Comfortable using iPads; experience with Athena EHR a plus
• Vehicle: Willingness to travel within assigned region (mileage/rental reimbursement available)
• Glove Preference: Nitrile or Latex (please specify size)

Compensation & Perks:

• Training: Paid at $50/hour for module completion
• Travel: Flight, hotel, and car rental/mileage reimbursement included
• Equipment: Medical equipment provided (excluding clothing)

Coverage Areas:

You must be willing to travel across Michigan to complete assignments as needed. Primary launch area is Kent County, with potential reassignment to other regions depending on member volumes.
