? Job Title: Nurse Practitioner – Assisted Living / Geriatric Care

Location: Klamath Falls, Oregon
Schedule: Monday–Friday, 8 AM–5 PM
Work Environment: Assisted Living, Post-Acute Rehab, Memory Care
Type: Full-time or Part-time (Flexible hours)

?‍⚕️ Key Responsibilities

Provide primary care services to geriatric patients in assisted living and post-acute settings.
Conduct patient assessments, develop care plans, and manage chronic and acute conditions.
Collaborate with facility staff including nurses, therapists, dieticians, and social workers.
Maintain accurate documentation using Epic EMR.
Respond to emergent needs and assist in urgent clinical situations.
Build long-term relationships with patients and families.

? Qualifications

Active, unrestricted Oregon NP license (or ability to obtain).
Board certified or board eligible.
Background in Family, Adult, or Geriatric care preferred.
New graduates are welcome to apply.

? Benefits

Salary: $110,000–$155,000/year (based on experience)
No weekends or on-call requirements
Relocation/sign-on bonus up to $5,000
CME allowance: $2,000/year
License renewal reimbursement
401(k) with 3% company match

Up to 200 hours PTO + 40 hours sick time
