Full-Time Primary Care Nurse Practitioner

We are seeking a compassionate and skilled Nurse Practitioner to join our primary care team, delivering comprehensive, patient-centered care to individuals across all age groups.

Key Responsibilities:

Conduct routine physical exams and preventive health screenings.
Diagnose and manage chronic conditions such as hypertension, diabetes, and asthma.
Provide acute care for minor illnesses and injuries.
Order and interpret diagnostic tests and lab results.
Develop and manage individualized treatment plans.
Educate patients on health maintenance, disease prevention, and lifestyle modifications.
Collaborate with physicians and specialists to coordinate care.

Qualifications:

Master’s Degree from an accredited Nurse Practitioner program.
Active NP license in New York (or willingness to obtain).
Minimum 1 year of experience in primary care, family practice, or internal medicine.
Board Certification required.
BLS and ACLS certifications preferred.
Bilingual (Spanish) a plus.
