Exciting News! We are now offering a sign-on bonus for both full-time and part-time Licensed Practical Nurses (LPNs). Plus, our evening shift differentials have increased! This is a great opportunity to earn more while working evening hours, with both 8-hour and 12-hour shifts available.

Position Summary

Provide essential nursing care by assisting in the assessment of residents upon admission and throughout their stay. You will be expected to remain informed and effectively communicate any significant changes in residents' conditions to the appropriate interdisciplinary team members. Your role will also involve documenting assessments accurately, seeking guidance when needed, and participating in the care planning process, ensuring updates are made accordingly. Implement necessary nursing interventions, including medication administration and treatments, while ensuring safety and timeliness.

Your critical thinking skills will be utilized to address any changes in residents' conditions, notifying physicians using the SBARR communication tool when required. Maintain composure in crisis situations, monitor care plans, identify problem areas, and actively engage in quality assurance processes. Remain attentive to residents’ behavioral changes and assist with daily activities as needed. Commitment to continuous education is essential, with a minimum of 12 hours of nursing-related training required.

Qualifications

• Education from an accredited nursing program resulting in a Licensed Practical Nurse certification
• Current LPN license in the state of employment
• CPR and AED certification required, or willing to obtain within six months
• IV certification required, or willing to obtain within six months
• Preferred long-term care experience
• Able to assess basic nursing care needs of geriatric residents
• Excellent communication skills for interacting with residents, visitors, and staff
• Strong time management and organizational abilities
• Physical capability to stand and walk for extended periods
• Ability to lift heavy objects and assist with patient transfers up to 200+ lbs using proper techniques
• Capability to push and pull equipment exceeding 40 lbs with assistance
• Manual dexterity and coordination for patient care and equipment handling
• Visual acuity to distinguish colors
• Able to perform necessary documentation both electronically and in writing
