Position Overview

As a Licensed Practical Nurse (LPN), you will play a crucial role in providing compassionate nursing care to our residents while supervising certified nursing assistants. Our goal is to create a warm and supportive environment in line with federal, state, and local regulations, as well as our facility’s policies and procedures.

Key Responsibilities

• Lead the day-to-day activities of certified nursing assistants to ensure the highest quality of resident care.
• Implement a holistic nursing model to cater to the emotional and physical well-being of residents.
• Conduct resident admissions, discharges, and transfers with professionalism and care.
• Monitor changes in resident health status and communicate effectively with physicians and family members.
• Transcribe and execute physician's orders with precision, maintaining thorough documentation.
• Administer medications and perform treatments as prescribed, while observing for any adverse effects.
• Document and address wound care needs in accordance with medical guidelines.
• Facilitate follow-up appointments and transportation for residents as needed.
• Provide mentorship and leadership to nursing staff on the unit.
• Perform essential nursing procedures, including colostomy changes, difficult wound care, tube feedings, and IV administration.
• Regularly assess that residents' needs are met and staff are fulfilling their responsibilities.
• Collaborate with the interdisciplinary team to ensure comprehensive care for all residents.
• Report and document any incidents or unusual occurrences promptly.
• Maintain an adequate supply of medications and nursing equipment.
• Update and manage care plans proactively.
• Engage in survey and quality assurance processes as required.
• Promote infection control principles and rectify any observed practices not adhering to them.
• Take action on any allegations of abuse or neglect, ensuring resident safety at all times.
• Complete other assigned duties as necessary while adhering to all policies and procedures.

Qualifications

Education: Nursing Degree from an accredited institution.

Licenses/Certifications: Current unrestricted LPN license in the state of practice and a valid CPR certification.

Experience: At least 1 year of clinical experience, preferably in a geriatric setting.

Skills & Abilities:

• Exemplary compassion and empathy when interacting with residents.

Compensation: Starting at $40.00/Hr, potentially up to $44.00/Hr based on experience.
