Position Summary

As a Licensed Practical Nurse (LPN), you will play a vital role in delivering high-quality nursing care to residents while also providing health-related consultation within the community.

Essential Functions, Key Duties, & Responsibilities

• Guide daily activities of nursing assistants and resident care associates, ensuring compliance with federal, state, and local regulations as well as established company policies.
• Implement a holistic care approach tailored to the unique needs of residents.
• Ensure adherence to policies and procedures by nursing personnel.
• Assist in the admission, discharge, and transfer processes for residents.
• Monitor changes in residents' conditions, alerting physicians and families promptly, while keeping accurate documentation.
• Transcribe physician orders into medical records and carry out those orders diligently.
• Administer medications as prescribed, monitoring for any adverse reactions.
• Perform and document wound treatments as per physician instructions.
• Coordinate follow-up appointments and transportation for residents, as needed.
• Provide leadership and support to the nursing team assigned to your unit.
• Deliver direct nursing care skills, including colostomy changes, tube feedings, wound care, suctioning, and IV administration, following prescribed guidelines.
• Conduct regular rounds to confirm that residents' needs are met and that staff are fulfilling their responsibilities.
• Collaborate with interdisciplinary team members to ensure comprehensive care for residents.
• Report any incidents or unusual situations to the leadership team promptly.
• Maintain adequate stock of medications and supplies, communicating needs as necessary.
• Initiate and review care plans regularly to meet residents’ evolving needs.
• Participate in the survey process during inspections.
• Engage in Quality Assurance Performance Improvement (QAPI) activities, as assigned.
• Ensure compliance with infection control principles and address any noted deficiencies.
• Perform additional duties as needed to support the team.
• Adhere to all updated policies and procedures.

Position Requirements

Education: Nursing degree from an accredited college or university.

Licenses/Certifications: Valid and unrestricted LPN license in the practicing state.

Experience: A minimum of 1 year of nursing experience preferred.

Skills & Abilities:

• Demonstrated compassion and empathy in resident interactions.

Compensation ranges from a minimum of $35.00/hour to a maximum of $38.00/hour.
