Licensed Practical Nurse (LPN) - New Compensation Package

Position Summary:

As a Licensed Practical Nurse (LPN), you will play a vital role in delivering exceptional nursing care to our residents while addressing health-related concerns in the community.

Essential Functions, Key Duties, & Responsibilities:

• Guide the daily activities of nursing assistants and care associates, ensuring compliance with federal, state, and local regulations.
• Apply a holistic nursing model to promote comprehensive care for residents.
• Ensure adherence to company policies and procedures by nursing personnel.
• Assist with the admission, discharge, and transfer of residents as necessary.
• Monitor changes in resident health and communicate with physicians and families as appropriate.
• Transcribe and execute physician orders accurately.
• Administer medications following physician guidelines and observe for any adverse reactions.
• Perform wound care treatments as prescribed and document changes in condition.
• Coordinate follow-up appointments and transportation for residents as needed.
• Provide leadership and support to nursing staff in the unit.
• Deliver direct care services including colostomy management, tube feeding, wound care, and IV administration.
• Conduct rounds to ensure resident needs are met and duties are performed correctly.
• Collaborate with interdisciplinary teams to achieve holistic resident care.
• Report any incidents or unusual occurrences to leadership.
• Maintain sufficient stock of medications and supplies, alerting necessary personnel as required.
• Initiate and review care plans regularly.
• Participate in survey processes during inspections.
• Engage in quality assurance activities when designated.
• Adhere to infection control protocols and assist in addressing any deficiencies.
• Perform additional duties as assigned.
• Comply with all relevant policies and any updates.

Position Requirements:

Education: Nursing degree from an accredited institution.

Licenses/Certifications: Current and unrestricted LPN license in the practicing state.

Experience: 1+ years of nursing experience preferred.

Min: USD $35.00/Hr. Max: USD $38.00/Hr.
