Key Responsibilities:

• Deliver direct patient care according to personalized care plans and physician orders.
• Monitor vital signs, symptoms, and responses to treatments; document patient behaviors.
• Work closely with Registered Nurses to report findings and implement patient care plans.
• Build meaningful relationships with patients by taking time to understand their emotional, physical, and mental needs.
• Celebrate and support patients’ victories throughout their recovery journey.

Qualifications:

• Active LPN/LVN licensure as required by state regulations.
• CPR certification.
• Preferred one year of experience in a medical-surgical or general hospital setting.
• Rehabilitation experience is a plus.
