POSITION SUMMARY

This position involves providing management and support for patients at Williamson Medical Center. You will offer short-term counseling and crisis management for patients and families dealing with adjustment to illness, lifestyle changes, or financial concerns stemming from their condition. Your role will include psycho-social assessments, discharge planning, patient and family education, and effective collaboration with medical professionals and community agencies.

POSITION REQUIREMENTS

Formal Education / Training:

• Must possess licensure in social work. LCSW preferred.
• Hold a Master’s degree in Social Work or a comparable degree.

Workplace Experience:

• Experience in a hospital or clinical setting is required.

Equipment and Skills Training:

• Proficient in using computer systems, Meditech, and Allscripts Care Management.
• Strong verbal and written communication skills are essential.

Physical Environment:

• Ability to work in office and clinical settings, with potential exposure to hazardous materials and infectious fluids.

Physical Effort:

• Must be able to walk short distances and lift 10-15 lbs. Visual and hearing acuity is required.

PERFORMANCE STANDARDS

1. Conduct psychosocial assessments and crisis management for patients within 24 hours of referrals, documenting all findings accurately.
2. Maintain efficient time management, ensuring prioritization of tasks and responding to referrals in a timely manner.
3. Identify discharge needs within 48 hours of admission for patients without consults, with proper documentation.
4. Collaborate with healthcare team members to develop and implement effective discharge plans, ensuring appropriate resource allocation.
5. Foster positive relationships with community agencies and healthcare teams through clear communication.
6. Deliver accurate patient information to post-discharge providers and assist in managing discharge appeals and payer denials.
7. Document discharge dispositions accurately within three days post-discharge.
