Position Summary
As a Licensed Medical Social Worker, you will provide essential support and management for patients at Williamson Medical Center. This role involves short-term counseling and crisis management for patients and families dealing with the challenges of illness, lifestyle changes, and financial concerns. You will be responsible for conducting assessments, developing discharge plans, and collaborating with healthcare teams to ensure comprehensive patient care.

Key Responsibilities:

1. Provide psychosocial, crisis, and substance abuse assessments, along with discharge planning for patients requiring immediate assistance.
2. Set priorities effectively, managing the workload efficiently, and responding to referrals within a 24-hour window.
3. Screen patients for discharge needs within 48 hours of admission and document findings appropriately.
4. Collaborate with healthcare team members and families to create tailored discharge plans, including post-discharge resources.
5. Maintain positive relationships with community agencies, ensuring timely communication of patient information.
6. Assist in the appeal processes for discharge decisions and payer denials.
7. Document discharge dispositions promptly and accurately.

Position Requirements:
Candidates must hold a Master's degree in social work or a related field and have licensure by the Tennessee Health Related Boards in Social Work (LCSW preferred). A background in a hospital or clinical setting is required, along with strong verbal and written communication skills. This role demands a commitment to patient-centered care, team collaboration, and effective time management.
