Are You Ready to Become the Lead Case Management Specialist You Aspire to Be?

In this role, you will:

• Perform advanced case management tasks with expertise.
• Understand and analyze patient outcomes alongside departmental objectives.
• Evaluate length of stay, costs, outliers, and discharge planning challenges.
• Assign and manage patient caseloads for optimal service delivery.
• Lead interdisciplinary teams and partner with various departments and medical staff.
• Mentor and support new case management staff or students through onboarding and training.
• Stay updated on performance improvement programs and relevant standards, including CMS and Joint Commission.
• Oversee departmental operations in the absence of the Director of Case Management.

Your Qualifications:

• Licensure: CCM or ACM certification is essential.
• For RNs: Must hold an unencumbered nursing license in the relevant state or from a compact state.
• For other healthcare professionals: An active license in a relevant field is required based on education.
• Minimum Experience: 2 years of case management experience, particularly in Utilization Review and Discharge Planning within rehab settings.
• For RNs: An associate degree in nursing (AND or ASN) is required, with a BSN preferred. Diploma is acceptable only in certain states.
• For non-RN roles: A minimum of a bachelor's degree in a relevant field is required, with a master's preferred.
