POSITION SUMMARY - LCSW Social Worker
POSITION DUTIES - Work alongside our RN Case Managers. Work in the inpatient or emergency department. Assess patients and/or patient's family, caregivers, and/or legal representatives and arranges for needed interventions. Plan care-focused interventions to help patients cope with physical, social, emotional, economic, and environmental concerns and provides treatment planning for support.
MINIMUM REQUIRED QUALIFICATIONS - LCSW - licensed in CO
Master's degree in Social Work
Flu vaccine
3+ years of clinical experience in the hospital setting
Discharge planning experience
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - Days with rotating weekends (36-40hrs/wk)
SYSTEMS - Meditech
START DATE - ASAP
