Hybrid Social Worker - MSW

As a Social Worker, you will provide vital social work services to patients and their families. You will collaborate with healthcare teams to educate patients and families about benefits, community resources, and counseling referrals. Your role includes triaging referrals and advocating for patients in a sensitive and supportive manner.

This is a hybrid position requiring occasional travel to our office locations, clinics, or contracted locations.

Primary Responsibilities:

• Assess patient and family psychosocial needs, creating care plans in collaboration with healthcare providers and departmental teams.
• Link patients and families to appropriate community resources, including financial assistance, transportation, support groups, and other services.
• Provide inpatient support, assist in discharge planning, and facilitate a smooth post-discharge transition, while monitoring hospital stay duration.
• Implement brief solution-focused therapy and harm reduction strategies when appropriate.
• Document patient/family evaluations, treatment plans, interventions, and progress accurately in medical records.
• Serve as an advocate for patients, coordinating care with physicians, families, insurance representatives, and community agencies.
• Conduct discharge planning that aligns with the treatment plan's identified goals.

Required Qualifications:

• Master's degree in Social Work (MSW), Psychology, Counseling, or a related human services graduate degree.
• 2+ years of experience in a medical setting as a Social Worker.
• Familiarity with medical terminology.
• Valid Driver's License and access to reliable transportation.

Preferred Qualifications:

• Experience in skilled nursing facilities or palliative care.
• Experience working in multi-disciplinary settings.
• Experience in managed care environments.
• Previous case management experience.
