As a Hospice RN Case Manager, you'll play a vital role in this mission. You'll guide patients and families through life's most meaningful moments while helping develop a team of compassionate caregivers who make an extraordinary difference every day.

Schedule: Full time, Monday through Friday 8a-5p

What You'll Do as a Hospice RN Case Manager

• Conduct patient assessments to identify physical, psychosocial, and environmental needs
• Develop and implement individualized care plans with the interdisciplinary team
• Monitor changes in patient conditions and communicate updates to the care team, physician, and family
• Educate and support patients, families, and caregivers in following the hospice plan of care
• Provide teaching on hospice philosophy, pain control, and symptom management in various care settings (home, nursing facilities, assisted living, etc.)
• Offer compassionate support at end-of-life and assist with bereavement needs

What We're Looking For

• Graduate of an accredited nursing program (Diploma, Associate, or Bachelor's)
• Current Registered Nurse (RN) license in the state of practice
• 1+ year of experience in a clinical care setting (hospice or home health preferred)
• Ability to travel (50% of the time) with reliable transportation, valid driver's license, and auto insurance
• Physical ability to sit, stand, bend, lift, and move intermittently (up to 50100 lbs)
