Compassionate Nurse Practitioner - Inpatient Hospice & Palliative Care Findlay, OH

Position Highlights:

Focus: Inpatient-based palliative and hospice care (75% inpatient / 25% outpatient)
Schedule: Monday-Friday, 8:00 AM-5:00 PM - average 6 patients/day
Setting: Primarily hospital-based care with collaboration across the care team
Responsibilities:
Manage symptoms and chronic diseases independently
Lead advanced care planning and goals-of-care conversations
Round on inpatient hospice patients daily
Work closely with hospitalists; admissions handled by hospitalist team
Open ICU environment - not required to manage ICU patients
Participate in shared on-call schedule

Ideal Candidate:

Passionate about hospice, palliative care, and geriatrics
Prior experience as an NP in acute care, palliative care, or hospice
Confident practicing autonomously in a hospital setting
Not accepting locums or new grads at this time
