In this fulfilling role, you will:

• Provide personal care and housekeeping assistance to clients in their homes following the established care plan.
• Assist with personal hygiene, grooming, and nutritional meal preparation.
• Monitor and report any changes in clients' conditions to the care coordinator or supervisor.
• Perform routine housekeeping and home management tasks.
• Document each client's care on the care plan and ensure timely submission.
• Respond compassionately to the complex needs of terminal clients, including providing end-of-life care.
• Assist clients with routine therapy exercises.

Qualifications:

• Education: High School diploma or equivalent preferred.
• Licensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services required.
• Experience: Six months experience as Home Care Aide/CNA in a home care agency preferred.
