Qualifications:

• A heart to serve patients and families with the utmost dedication
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
• Experience: Minimum of 2 years within the last 5 years in chronic disease management or long-term care. Hospice experience preferred.
• Additional Requirements: Reliable transportation, ability to perform physical tasks including sitting, standing, bending, and lifting up to 25 lbs.
