Key Responsibilities:

• Provide personal care and assistance in housekeeping to clients in their homes, following the established care plan.
• Assist with personal hygiene and grooming needs.
• Prepare meals tailored to clients' nutritional requirements.
• Monitor and report any changes in clients' conditions to the client care coordinator or supervisor.
• Perform routine housekeeping and home management tasks.
• Document each client's care accurately and submit reports in a timely manner.
• Respond compassionately to the varied needs of terminal clients, including offering end-of-life care.
• Assist clients with routine therapy exercises.

Qualifications:

• Education: High school diploma or equivalent preferred.
• Licensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services required.
• Experience: Six months experience as a Home Care Aide/CNA in a home care agency preferred.
