Key Responsibilities:

• Facilitate effective communication between patients and the interdisciplinary team.
• Develop and maintain therapeutic relationships by providing education and encouraging self-advocacy and independence.
• Utilize advanced technology to document skilled services rendered.

Qualifications:

• Current and valid CT LPN license.
• A minimum of one year of LPN experience is required; homecare experience is preferred.
• Valid driver's license and dependable transportation are required.
• Proficiency with computers is necessary.
