Home Health Nurse Practitioner

Now Hiring: Nurse Practitioner/Physician Assistant Home-Based Primary Care (Fargo, ND Region)

We are a non-profit, community-owned organization with 40+ years of experience in delivering compassionate, patient-centered care across North Dakota and Minnesota. Our team is supported by full-time, dedicated physicians and nurse practitioners who specialize in hospice and palliative medicine.

Location: Fargo, ND
Schedule: Flexible scheduling with autonomy to tailor visits to patient needs (M-F)

Team Meetings: On-site once weekly

Day-to-Day Responsibilities:

• 50/50 split between home visits and assisted living facility visits

• Expectation to see 4 8 patients per day depending on acuity and travel time

• Collaborate with interdisciplinary care teams

• Deliver exceptional, mission-aligned care to patients in a variety of home-based settings

Requirements:

• Minimum of 3 years of NP/PA experience in primary care with a focus on geriatrics

• Strong preference for prior home-based care experience

• Current licensure in North Dakota and Minnesota (or ability to obtain)

• Active DEA license

• Valid driver s license and auto insurance

• Ability to travel regularly within the designated territory

• Experience in hospice and/or palliative care is highly preferred
