Schedule : Sunday through Saturday - Day and evening shifts available. Must be available to work at least 1 - 8 hour shift every 2 weeks with 1 weekend shift per month.

This position is ideal for nursing students looking to gain valuable patient care experience while making a positive impact on the lives of our clients. You will provide essential care, following each client's personalized plan of care, and demonstrate compassion in every interaction.

ABOUT ELDERONE

ElderONE is an affiliate of Rochester Regional Health and the only PACE program in the area, dedicated to serving Monroe, Ontario, and Wayne counties. We offer an innovative alternative to long-term nursing home placement by providing an entire continuum of medical care and support services to seniors 55 and older, helping them maintain independence in the comfort of their homes without compromising their health or safety.

KEY RESPONSIBILITIES

• Assist in creating and implementing a comprehensive plan to meet patient needs, both daily and for the long term.
• Document patient activities and general condition regularly.
• Support clients in their daily routines, including personal care, light housekeeping, and meal preparation.
• Complete aide activity sheets accurately and submit them in accordance with departmental policies.
• Participate in a minimum of 12 hours of annual in-service education/competencies.
• Communicate any changes in patient condition to the appropriate staff promptly and document on the Aide Activity sheet.
• Keep family members and staff informed about client activities and concerns.
• Perform other duties as required.

REQUIRED QUALIFICATIONS

Nursing students must have documented success in coursework covering home health aide tasks completed within the past 24 months. An official transcript is acceptable for verifying completion of Fundamentals in Nursing.

LICENSURE/CERTIFICATION

A valid NYS Driver's license is preferred. You must pass a written and one-day HHA Skills competency program provided by Rochester Regional Health and obtain active New York State HHA certification within 4 days of hire.

PHYSICAL DEMANDS

The role requires medium work, including lifting 20 to 50 pounds occasionally and performing activities that require frequent walking, standing, or squatting.

This position offers a pay range of $20.60 per hour. The listed base pay range is a good faith representation of potential base pay for a successful applicant, which may be modified based on experience, qualifications, specialty, internal equity, location, and contract terms.
