Your Specific Responsibilities:
This is a highly responsible and independent position within the Health Promotion (HP) Program, serving as the Healthy Communities (HC) of Florida liaison for the Florida Department of Health in St. Lucie County (DOH-St. Lucie). The role ensures completion of all activities and reporting requirements outlined in the Healthy Communities of Florida Workplan, while also supporting initiatives tied to the Community Health Improvement Plan (CHIP) for St. Lucie County.
The primary focus is on developing and implementing community-based strategies to reduce the burden of chronic disease. This includes executing activities from both the Healthy Communities Manual and Workplan and the CHIP, with an emphasis on chronic disease prevention and health promotion.
Key areas of responsibility include advancing Policy, System, and Environmental (PSE) changes related to nutrition and physical activity in early childcare and education (ECE), school, and community settings, as well as promoting worksite wellness within DOH-St. Lucie and local businesses.
This position requires expertise in program planning and evaluation, public speaking, coalition building, and marketing/public relations.
Florida's Healthy Communities Workplan - Policy, System, and Environmental (PSE) Changes in ECE and School Settings
- Utilize the most current Healthy Communities of Florida (HC) Program Manual and Workplan to develop and implement local plans that engage early childcare education (ECE) and school settings in nutrition and physical activity initiatives. Current initiatives this position is responsible for:
- Go NAPSACC, Go Nutrition and Physical Activity Self-Assessment for Child Care; increase engagement and completed modules,
- Alliance for a Healthier Generation; assist and provide continual follow-up with school district in completing the Thriving Schools Integrated Assessment and select 1 action item each to address physical activity and nutrition,
- Identifying and implementing a local PSE promising practice focusing on ECE and/or school setting.
- Maintain active participation in the School Health Advisory Council (SHAC)
- Ensure timely completion and submission of all required reports to the Florida Department of Health (FDOH).
- Maintain regular communication with the program manager and Health Promotion team regarding ongoing activities and projects.
- Utilize the most current Healthy Communities of Florida (HC) Program Manual and Workplan to develop and implement local plans that engage community settings in nutrition and physical activity initiatives. Current initiatives this position is responsible for:
- Using the PACE-EH methodology to assess environmental health needs of the community and facilitate the resulting improvement activities. PACE-EH may require partnering with other DOH-St. Lucie departments and community partners, this position should lead and monitor progress.
- Identifying and implementing a local PSE promising practice focusing on a community setting.
- Collaborate with local organizations and stakeholders to support chronic disease prevention efforts.
- Provide technical assistance and resources to community leaders to enhance program reach and effectiveness.
- Participate in public events, outreaches, and campaigns to raise awareness and encourage community engagement.
- Identify opportunities for improvement and lead efforts to implement PSE changes that enhance employee wellbeing at DOH-St. Lucie.
- Develop, launch, and sustain a comprehensive worksite wellness plan for DOH-St. Lucie employees.
- Partner with businesses of varying sizes to share best practices and resources for worksite wellness.
- Report activities and progress to the CHIP lead and ensure proper documentation for inclusion in CHIP reports.
- Submit monthly updates to the program manager detailing worksite wellness initiatives.
- Prepare, route, and submit the Healthy Communities bi-annual report, annual narrative, and success story for DOH-St. Lucie.
- Create and route for approval, or identify, awareness campaigns on the role of healthy eating and physical activity in preventing and managing chronic diseases such as diabetes, cancer, and heart disease.
- Use an annual editorial calendar to plan awareness marketing, including flyers, presentations, and social media content.
- Lead or participate in community outreach events to engage stakeholders on chronic disease prevention, HP initiatives, and HC projects.
- Manage purchasing of approved educational and promotional materials related to HC projects and chronic disease prevention.
- Actively participate in the Healthy St. Lucie Coalition and other community coalitions to promote the goals of the HC of Florida, CHIP, and the Community Health Liaison's Health Improvement Plan.
- Collaborate with partners and other Health Promotion programs to maximize community impact.
- As needed, assist in drafting written summaries for CHIP quarterly action plans and the annual progress report.
- Attend all relevant meetings, including:
- Statewide HC and individual program meetings
- Healthy St. Lucie Coalition meetings
- DOH-St. Lucie Health Promotion meetings
- Community events and other assigned programmatic gatherings
- Promote additional health programs including Tobacco Free Florida, and health screenings.
- Serve as the back-up in the absence of the for the National Diabetes Prevention Program lead.
- Obtain certification as a lifestyle coach through "Prevent T2 for All."
- Assist in data collection and reporting.
- Support recruitment and promotion efforts for the DPP program.

Qualifications:
Minimum:
- Florida Department of Health is required to work before, during, and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelter or performing other emergency duties including but not limited to, response to or threats involving any disaster or threat of disaster man-made or natural.
- Driver's license, if out of state, a Florida Driver's license is required within 30 days of hire
- Required to travel 0-25% of time per week.
- Bachelor's or master's degree in public health, nutrition, health education, education, human services, or a related field.
- Three years of experience working in health education
- Certified Health Education Specialist
- Bilingual or multilingual in English/Spanish and/or Haitian Creole
- Proficiency in Microsoft Office programs including Word, Excel, PowerPoint, Teams, and Outlook.
