Your Role as a Case Manager:

• Collaborate with an interdisciplinary team to tailor treatment plans based on patient needs and preferences.
• Coordinate discharge planning and ensure all contingency plans are in place.
• Monitor patient experiences and participate in weekly team conferences to optimize treatment plans.
• Document all case management actions and maintain compliance with regulations and company policies.
• Educate patients and their families about rehabilitation processes and the Case Manager’s role.
• Assist patients in procuring necessary resources to ensure smooth transitions from discharge.

Qualifications:

Licensure/Certification:

• Must be able to complete assessments independently based on your discipline (e.g., RN, SW, OT, PT).
• If licensure is required for your discipline, an active license is a must.
• Certification in Case Management (CCM® or ACM) is required or must be obtained within two years of position acceptance.

Minimum Requirements:

• An Associate Degree in Nursing (for RNs) is required; BSN preferred.
• Other eligible healthcare professionals should have a minimum of a bachelor’s degree, with a graduate degree preferred.
• Two years of experience in rehabilitation preferred.
