Become the Case Manager You Aspire to Be
- Collaborate with an interdisciplinary team to develop treatment plans tailored to patient needs and preferences.
- Coordinate with the team to establish discharge plans and contingencies.
- Participate in enhancing the patient discharge experience.
- Monitor patient experience across various metrics: quality, timeliness, and appropriateness of services.
- Lead weekly team conferences and oversee treatment plan modifications.
- Maintain records through case management addendums and required documentation.
- Stay informed on regulations, company policies, and operational standards.
- Analyze case management reports and implement appropriate actions.
- Comprehend commercial contract stipulations and payor requirements.
- Engage in Acute Care Transfer (ACT) meetings to identify trends and improve outcomes.
- Meet with patients and families within 24 hours of admission to establish connections.
- Conduct assessments and complete necessary documentation within 48 hours of patient admission.
- Educate patients and families on rehabilitation processes and the role of the Case Manager.
- Facilitate family conferences as needed.
- Assist patients in timely resource planning to prevent discharge delays.
- Ensure regulatory compliance in ordering orthotics and prosthetics.
- Make timely referrals and documentation for post-discharge support.
- Verify the accuracy of discharge and payor-related information in patient records.
- Engage in the utilization review process, including data collection and trend analysis.
- Participate in case management on-call scheduling as necessary.
Qualifications
License or Certification:
- Must be qualified to independently perform assessments within the scope of your clinical discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
- If state licensure is required, you must hold an active license.
- Must meet eligibility requirements for CCM® or ACM certification upon entry into the role or within two years of commencing this position.
- CCM® or ACM certification is required or must be obtained within two years of assuming the position.
Minimum Qualifications:
- For Nursing, must possess at least an Associate Degree in Nursing, with RN licensure; BSN preferred.
- For other eligible health care professionals, a minimum of a Bachelor’s degree is required, with a graduate degree preferred.
- Preferred: 2 years of rehabilitation experience.
