Your Role as a Case Manager

• Collaborate with an interdisciplinary team to guide treatment plans based on individual patient needs.
• Coordinate discharge planning and ensure a seamless transition from inpatient care to home.
• Monitor the patient experience and facilitate regular team meetings to adjust treatment plans as necessary.
• Document all case management processes and maintain compliance with regulations and best practices.
• Educate patients and their families on rehabilitation options and resources available to them.

Qualifications to Succeed

• Must be qualified to independently assess patients and hold an active license in your discipline.
• CCM® or ACM certification preferred or required within two years of employment.
• For nursing candidates, a minimum of an Associate Degree in Nursing is required; a BSN is preferred.
• Other candidates must possess a minimum of a bachelor’s degree in their respective fields.
• At least two years of experience in rehabilitation is preferred.
