Your Role as a Case Manager

  • Collaborate with an interdisciplinary team to guide treatment plans tailored to patient needs.
  • Work with the team to create tentative discharge plans and backup strategies.
  • Facilitate the planning and execution of patient discharge experiences.
  • Monitor patient experiences, including quality, service appropriateness, and expectations.
  • Lead weekly team conferences to coordinate all treatment plan adjustments.
  • Complete necessary case management documentation and addendums.
  • Stay updated on regulations, company policies, and department operations.
  • Review case management reports and plan appropriate actions based on Key Care Indicators.
  • Understand payor requirements and ensure compliance with recertification needs.
  • Attend Acute Care Transfer meetings to analyze trends and collaborate to reduce ACTs.
  • Meet with patients and their families within 24 hours of admission.
  • Assess goals and complete the initial case management addendum within 48 hours of admission.
  • Educate patients and families about rehabilitation and the Case Manager's role.
  • Schedule and facilitate family conferences as needed.
  • Assist patients with acquiring necessary resources to avoid discharge delays.
  • Ensure compliance with regulations regarding orthotics and prosthetics.
  • Timely referrals to post-discharge providers and physicians with thorough documentation.
  • Verify the accuracy of discharge and payor-related patient record details.
  • Participate in utilization review processes for data collection and action resolution.
  • Join the on-call schedule for case management as needed.

Qualifications

License or Certification:

  • Must be qualified to independently complete assessments aligned with your discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
  • If licensure is necessary for your discipline in the state, you must hold an active license.
  • Eligibility for CCM® or ACM certification upon entering the position, or within two years.
  • CCM® or ACM certification required or must be obtained within two years of assuming the Case Manager II position.

Minimum Qualifications:

  • For Nursing roles, a minimum of an Associate Degree in Nursing is required; RN licensure is mandatory, with BSN preferred. A diploma is acceptable where applicable.
  • Other licensed or certified health care professionals must hold a bachelor’s degree, with a graduate degree preferred.
  • Two years of rehabilitation experience is preferred.
