The Aftercare Coordinator is responsible for assisting with multiple clerical functions within the Clinical Services Department. The Aftercare Coordinator will assist with discharge planning by helping social worker make collateral contacts and arrange discharge plans/linkage for patients with aftercare services. The Aftercare Coordinator assist the social workers with coordinating court hearing for patients, act as liaison for website access to outside agencies, and assist with staffing the social services department..

Education: Bachelor's Degree Required.

Experience: 1 - 3 years Clinica experience in psychiatric setting preferred.
